Please note that the following documentation must be received by us before 4.00pm on Monday 27 September 2010 and that regretfully, we are unable to accept late applications.

• Application/Declaration form (ref AD 2011), fully signed and completed
• a photocopy of your child’s Year 5 Primary School report, including details of his/her attendance over the past year
• any relevant special needs information (see page 3 of the Rules of Admission).

Upon receipt of your completed Application/Declaration Form, we will send you details of your child’s assessment session.
